If I had to guess at your problem, I would guess that you were overdriving the hardware, voltage or current wise.  If you drive the pots or circuits with too high a voltage or current, they may appear to work for a while, but eventually they'll burn-through and fail.  Just a guess, but it would explain your symptoms.
