Who decide of the use of differents modes and how are decided codes (on code
wheels) ?

If flying in a civilian enviroment (usually all the time) you will set
assigned mode codes so air traffic controllers will see your target and be
able to identify it.  When under control of military awacs controllers you
will use mode 4, or the older mode 2.  In Vietnam, I once had Gen. Robin
Olds tell that the closer he got to high threat areas the more stuff he
turned off, and the iff was one of the first and the radio (not the
intercom) for the rear seater was next.  Even if not under atc control you
are still required to transmit certain codes to let the controllers know
about you.  Primary radar is not really used too much these days, as
secondary radar (iff returns) are much cleaner, show up better and have more
information.
